Declaration of Finance Form must be notarized?

I have just noticed that the Declaration of Finance Form mentions notarization at the end. I would like to ask if this is required if all funds are from MY personal bank accounts? I have had all the official bank statements in hand by the way. Thanks!

I believe you have to get notification. Even I live in the US, when I do some of my financial documents, I have to bring it to public notarization places to get it signed.
I think you can get it notarized for free by your bank. I know it's US banks do this for free.
